Access to Information and Disclosure

Payukotayno James & Hudson Bay Family Services provides access to information and disclosure to records pursuant to Part X of the Child, Youth and Family Services Act.

Former or Current Clients

If you are currently receiving services from Payukotayno or have in the past, and would like access to your records, please complete the form below. You will also be required to provide one copy of a valid government issued photo identification (front and back) or alternative along with your request. Once your identity is verified, any photo identification documents will be destroyed and will not be stored in any Information Management System.

Record Check

If you require a record check or would like to know if you have records with Payukotayno, please complete the form below.
